A registration fee must be included with your application. Please include a $20.00 fee if you are
registering for 3 events or less; please include a $30.00 fee if you are registering for 4 or more
events. Due to the time and expense of processing entries, this fee is NON-REFUNDABLE. This
fee provides the applicant with event participation, an Ozark Senior Olympics T-Shirt, and a
Dinner & Bingo Ticket (if reserved). Checks should be made payable to OZARK SENIOR OLYMPICS
and mailed with entry application to OZARK SENIOR OLYMPICS, 2725 N. Westwood Blvd. Poplar
